Consent Page

Research surveys must have a consent page; should be in a block by itself and contain only text or a yes/no question.  If a question is yes or no, you should provide a skip option for those who say they don't wish to participate in the survey.  You are presuming their consent if they continue if you have just texted. 

1. Copy and paste directly from Word; watch out for word codes. It could be necessary to remove the format in the rich text editor.
