Ways to include a consent page in a survey.
Consent Page
Research surveys must have a consent page; should be in a block by itself and contain only text or a yes/no question. If a question is yes or no, you should provide a skip option for those who say they don't wish to participate in the survey. You are presuming their consent if they continue if you have just texted.
- Copy and paste directly from Word; watch out for word codes. It could be necessary to remove the format in the rich text editor.
